چکیده

28 Objective: We developed and tested three dimensional (3-D) indices for quantifying 29 severity of deformational plagiocephaly (DP). 30 Design: We evaluated the extent to which infants with and without DP (as determined 31 by clinic referral and two experts’ ratings) could be correctly classified. 32 Participants: Infants ages 4-11 months, including 154 with diagnosed DP and 100 infants 33 without a history of DP or other craniofacial condition. After excluding participants with 34 discrepant expert ratings, data from 90 infants with DP and 50 infants without DP were retained. 35 Measurements: Two-dimensional histograms of surface normal vector angles were 36 extracted from 3-D mesh data and used to compute the severity scores below. 37 Outcome measures: Left Posterior Flattening Score (LPFS), Right Posterior Flattening 38 Score (RPFS), Asymmetry Score (AS), Absolute Asymmetry Score (AAS) and an approximation 39 of a previously described 2-D measure, the Oblique Cranial Length Ratio (aOCLR). Two40 dimensional histograms localized the posterior flatness for each participant. 41 Analysis: We fit receiver operating characteristic curves and calculated the area under the 42 curves (AUC) to evaluate the relative accuracy of DP classification using the above measures. 43 Results: The AUC statistics were: AAS=91%; LPFS=97%, RPFS=91%; AS=99%, and 44 aOCLR=79%. 45 Conclusion: Novel 3-D-based plagiocephaly posterior severity scores provided better 46 sensitivity and specificity in the discrimination of plagiocephalic and typical head shapes than 47 the 2-D measurements provided by a close approximation of OCLR. These indices will allow for 48 more precise quantification of the DP phenotype in future studies on the prevalence of this 49 condition, which may lead to improved clinical care. 50

Comparison of infant head shape changes in deformational plagiocephaly following treatment with a cranial remolding orthosis using a noninvasive laser shape digitizer.

Deformational Plagiocephaly (DP) is a multi-planar deformity of the cranium occurring either pre-or postnatally in infants. In the last decade, the incidence of DP has increased substantially due to a number of factors, including supine sleeping positioning to reduce Sudden Infant Death Syndrome and the use of child carriers that increase supine positioning. Clinical questions persist about whi...

Automatic 3D Shape Severity Quantification and Localization for Deformational Plagiocephaly.

Recent studies have shown an increase in the occurrence of deformational plagiocephaly and brachycephaly in children. This increase has coincided with the "Back to Sleep" campaign that was introduced to reduce the risk of Sudden Infant Death Syndrome (SIDS). However, there has yet to be an objective quantification of the degree of severity for these two conditions. Most diagnoses are done on su...

Craniosynostosis.

Skull deformity in infants continues to be a diagnostic and therapeutic challenge. Deformational plagiocephaly is a common and somewhat benign cause of skull deformity in infants that must be distinguished from the more serious craniosynostosis, which occurs alone or as a syndrome. Examining an infant's head from above can help the physician distinguish true lambdoid synostosis from deformation...

Long-term developmental outcomes in patients with deformational plagiocephaly.

OBJECTIVES To determine whether there was an increased rate of later developmental delay in school-aged children who presented as infants with deformational plagiocephaly without obvious signs of delay at the time of initial evaluation. METHODS A retrospective medical record review of 254 patients evaluated at the Craniofacial Center of the Children's Hospital and Regional Medical Center in S...

Head shape measurement standards and cranial orthoses in the treatment of infants with deformational plagiocephaly.

This review aims to determine how head shape is measured and describes the use of orthoses in the management of deformational plagiocephaly. A systematic review was conducted and papers published in English up to and including 2006 were sourced from nine databases. After initial screening, 20 papers were included; three literature reviews and 17 original papers. Of the original papers, eight co...
